You can obtain a copy of a prior year tax return by completing SC4506, Request for Copy of Tax Form or Tax Account Information. You can also send a written request containing the following information: your name, Social Security number, and, if you filed a joint return, the name and Social Security number of your spouse, the form number, the tax period or year, and your current address.
